Client Team Assignment

Overview

The Client Team Assignment form allows staff to assign each client to a team of providers.  The Team Definition itself is defined by the BHIS Avatar Team, please submit a Help Desk Ticket and include the Team Name(s) with a list of staff assignments and/or removals. 

 Adding and removing clients from a Team is the responsibility of the program.  Use the Client Team Assignment form to maintain client assignments.

Form Structure

Open the Client Team Assignment form to begin the process. 

 Once a client is selected either the current Team assignment will be displayed or the message, “Client is not assigned to any teams currently.”  

 The Client may then be added.

ADD/Remove Client

  1. Select Client – search for the client to be assigned

  2. Current Teams – this area will display the current assignment if already assigned

  3. Add or Remove from Team – Select Add or Remove options depending on requirement.

  4. Add to Team* – this will be enabled if the “Add” button was selected.  Select the team to add this client.

  5. Remove from Team* – this will be enabled if the “Remove” button was selected.  Select the team to remove this client.

  6. Episode to Add Remove – Once the Add or Remove option is selected, the episode must also be selected.

  7. File – Click the File button to save the changes.
