Document Scanning Headersheet Templates

Below are each of the templates to use as headersheets when using Perceptive batch scan functionality within Avatar. Please note that each of the templates contain pre-formatted barcodes for each of the approved document categories, however, you will still need to add the proper information and barcodes to cover client ID (CID) and episode ID (EID). To do that, you will need to follow the steps below or utilize the guide: Document Scanning - Creating Barcodes

Step-by-step guide

To complete a template to be ready for use as a headersheet:

  1. Start below the document scanning category that will already be there (text should be centered)
  2. Type your client ID information using the correct format (e.g. *CID-12345* with the asterisks included, you do not need leading zeroes)
  3. Type the episode ID information for the client using the correct format (e.g. *EID-4* with the asterisks included, non-episodic scans should be *EID-0*)

When done, you can print your headersheet and insert it in the correct place in your batch. You will need to change headersheets for each change in your batch: document category, client and/or episode. 

Any mistakes made in the initial scan can be corrected afterward in your review before submitting/saving your batch to Avatar. Please make sure to review thoroughly as corrections possible after a document has been saved to Avatar are extremely limited.

You can download each of the files below by clicking to open them, then clicking the Down Arrow towards the top right of the preview screen to download them. Note the barcode font doesn't show in the preview, but will when you open the document in Microsoft WordPad (recommended) or Microsoft Word.
