Treatment Plan error "Plan Data Cannot Be Found"

Problem

When writing a progress note and attempting to link the note to an item in the treatment plan, the below error can be encountered: 

Solution

This usually occurs for one of a few different reasons. If you go through the steps below, you should be able to pinpoint why the error is occurring. 

  1. Verify the correct plan version is being selected - there may be multiple selections in the "Select T.P. Version" field, so make sure you are selecting the proper Treatment Plan type from your progress note
  2. Verify the plan exists - from the Home or Chart View, look for the plan data itself to make certain it's been created for the client to whom you are charting
  3. Confirm the plan dates - a plan's interventions can only be linked to progress notes with a service date within its range, so within the treatment plan form, confirm the Plan Date and Plan End Date and make sure your service date is within that range

If any of those things are incorrect, check with the plan author or your program leadership for corrections. However, if they seem correct, contact the Help Desk to open a support case.
