Duplicate interventions on service or treatment plan report

Problem

Interventions on a treatment plan report duplicate (i.e. Telecare Individual Service Plan Report or other Treatment Plan Reports), despite the fact that on the actual plan itself the interventions are only listed a single time. 

Solution

This is caused by having more than one plan author on the plan setup page, as shown in the screenshot below:

To resolve this, make sure only one of the plan participants has "Yes (Y)" under the "Plan Author" column.
