Onboarding/Offboarding Practitioners (association and termination)

This article will go over onboarding and offboarding of practitioners in Avatar, with a focus on proper program association and termination of practitioners. As a reminder, practitioner accounts in Avatar are required for billing and necessary to create clinical documentation.

 Onboarding (new staff)

This will go over the process in Avatar; it is assumed you will have submitted a UAR for Avatar access if needed. This is for staff brand new to Telecare:

  1. Create the practitioner account for the staff using the Practitioner Enrollment form (a guide on using this form is in Relias).

  2. Make sure to check off the programs for which the staff is expected to provide services, including outreach programs if necessary. Only add the programs for which you are responsible.

  3. When completed, Submit the form.

Onboarding (existing staff)

For staff that are already working for Telecare and have an existing practitioner account:

  1. Search for and select the staff while launching the Practitioner Enrollment form.

  2. Add your program(s) in the Program Association field

  3. Submit when completed

Offboarding (staff working only for your program)

For staff working only for your program(s):

  1. Access the Practitioner Enrollment form and uncheck your program(s).

  2. Submit the form

  3. Use the Practitioner Termination form to enter a termination date (though not required, it is a good idea to check the box to hide staff from the Scheduling Calendar)

Offboarding (staff working for multiple programs)

For staff working for multiple programs (e.g teleclinicians, float pool RNs or others)

  1. Access the Practitioner Enrollment form and uncheck your program(s).

  2. Submit the form (no other action necessary)

If a staff person is terminated in error, you can use the Delete Last Practitioner Termination Date form to undo the termination
