Name Change - correction process (Update Client Data)

This article goes over the method to correct client/patient names in Avatar. Typically all staff have access to this form, so anyone can make this correction.

Update Client Data

  1. You can either launch the form directly and search the client on the Select Client screen, or search the client in the Client/Staff widget, first, and launch the form with the client highlighted in green

  2. Change the name in the Client Name field

    1. Note the format should be LASTNAME,FIRSTNAME MIDDLENAME (no space after the comma, but a space after the first name if you are adding a middle name)

  3. Submit to save your changes

Once an Admission is saved for a new client, the name cannot be updated within that form and the field will be grayed out
